What If I'm already a member of EoS Fitness?   
  • Enroll for the corporate membership online    
  • Email This email address is being protected from spambots. You need JavaScript enabled to view it. afterwards with the new agreement information and your current agreement is cancelled with no further billing   
  • The annual membership fee on the new agreement will be moved to the following year on the same month you paid this calendar year   
    • If you haven't paid the AMF this calendar year, it will come out with your first monthly invoice.    
  • Confirmation will be sent via email once processed.    
  
What if the enrolling member is a minor?   
  • Parent/Guardian enters all of the minor's demographic information EXCEPT Date of Birth  
  • Parent/Guardian enters their own DOB  
  • Parent/Guardian signs their name, not the minor's, when prompted to sign for the agreement

    • Must be 13 years old 
  • Confirmation will be sent via email once adjustment has been made

Regular Full-Time Employees are eligible to earn Paid Time Off (PTO) & Paid Holidays. Full Time Employees must maintain a minimum of 30 hours worked per week to remain eligible for PTO & Paid Holidays. Eligible Tenure is calculated on the basis of a "Benefit Year." A 12-month period calculated from the Employee’s Hire Date. Switching from Part Time to Full Time would be calculated based on the Total Hours Worked for an Adjusted Hire Date.

Employees may Rollover Un-Used PTO to the Max Limit of 120 hours PTO from one year to the next. Any PTO accrued beyond the max limit will be forfeited at end of the calendar year. Hrs paid at standard pay rate PRate when used.

 

Ex1: PRate $18.50 x 56 hrs PTO = $ 1036.00 + $18.50 x 56 hrs Hol = $ 1036.00 => $ 2016.00 => $1.00 Per Hr Benefit

Ex2: PRate $23.00 x 80 hrs PTO = $ 1840.00 + $23.00 x 56 hrs Hol = $ 1288.00 => $ 3128.00 => $1.50 Per Hr Benefit
